Call Assistant partially working

This a new phone system, and try as I might I cannot get the Call Assistant to work properly -- Either in house or for myself, a remote user.

It behaves quite oddly. It registers when I pick up a line, but does not register the disconnect of the line.

Does anyone have any suggestions?
