Setting up Allworx 48x into Key System

This is my first Allworx system, and I have learned a number of things on this forum, so thank you for your time and input.
I am at a point int he configuration where I don't know why it is not working.
This is my setup:
Host based Allworx 48x and 6/2 expander v7.4.9.5
23 9212L SIP phones and 1 9224
I have set up one CO line on port 1 and 1 9212L to test with. Dialing out works fine (dial 9 and it picks up the line and can make outgoing calls).
I have enabled line appearance on Port 1
I have set up a Generic User on extension 2999 that is not assigned a phone and I have routed all calls from this CO to this extension.
We will not be using Auto Attendent features or voicemail, so the extension call route looks like this:
First connection attempt:
Key System Ring Delay (Call Monitor) for 99 rings (Single Ring)
Finally:
hang up
I have a PFK on the 9212L programmed as a Line Appearance for CO 1.
Here is what happens:
I initiate an incoming call, and the PFK lights up flashing green (showing activity on the line). As soon as I push the PFK to answer or pick up the phone to answer, the 9212L flashes like it has taken control of CO1 and then immediately sends the PFK into a flashing red state. I can not assume the call by holding the button, pushing the button or any other means.
Surely I am missing something in the Key System template, but I don't know what it could be. This is my template that I copied from the original and then made some minor modifications:
