SIP setup

We are setting up new 48x with SIP trunk. I have been going through multiple sources trying to find the right answers. I am working getting in touch with allworx support direct, but figured I would try here for any helpful information.
According to our ISP the SIP device is a gateway and not Proxy.
If I setup a SIP gateway I can make and receive local calls fine. However, long distance does not work. It is getting rejected by the SIP provider because it is coming from the phones extension instead of the actual DID line. Therefore the SIP provider has no way to route it back. So the first solution would be if anyone knows how to set the handsets to use the DID number instead of their own extensions for external calls. I have manually set all caller IDs and it still does not resolve.
I also tried setting up a SIP proxy. With this setup I can receive calls but not make them, local or long distance. It gives an error that the handset does not have permission to use the services at 5900 which is the SIP proxy. I verified with our provider that those calls are not even making it to the SIP trunk, so the allworx is denying the access. So if that could be fixed I have noticed the proxy has some extra options for digits sent that might resolve the long distance problem.
The setup is per Allworx response is LAN Host mode. The LAN port is set to 192.168.2.254 and the WAN port is set to 192.168.3.254. The SIP trunk is plugged into the WAN and local phones on the LAN port.
Any advice would help for anyone with experience on setting up an allworx with SIP instead of PRI.
