After recovering from a disaster, we may have imported XML more than once, so we had duplicated users with a 00 added (usera00, userb00, etc).
We deleted the users, but then noticed duplicated extensions (extensiona00, extensionb00).
There is no button to delete these duplicated extensions, and they can't be updated, because the user connected to the extensions no longer exists. This is the error we get when attempting to update the duplicate extension:
Error updating profile for user in system database.

Phantom extensions, cannot delete

I also noticed that if I try to add a user that was deleted previously, say usera00, I get a message that the username exists in the database, but does not show up in the user list under business.
