On a new install my phone displays "Init/load failed".
What do I do?

Try resetting the phone to factory defaults. Using the config soft key scroll down to "Set factory defaults"

Stephen is right. Try resetting the phone back to factory defaults, as this usually means that the phone cannot find the AllWorx for some reason, usually network related. If you have static addresses/VLAN etc then they will have to be setup again as well.
